I understand that the term of office for City Council Member will be effective January 7, 2026 through January 2028.
Certification of Information and Qualifications – I hereby certify that the information above is true and correct to the best of my knowledge, that all the following statement regarding legal qualifications* are also true and correct, and that I give my consent to allow for the City to verify that the following statements are true and correct:
- Must be a registered voter in Mapleton City (UCA §20A-2-101)
- Must have resided within Mapleton City for 12 consecutive months immediately before the date of the appointment
- Maintain a principal place of residence within the municipality, and within the district that the elected officer represents, during the officer's term of office
- Pay a $35 filing fee
- Not convited of a felony, treason, or crime relating to elections (UCA §20A-2-101.3 or 20A-2-101.5)
- Cannot have been declared mentally imcompetent
